 <title>Acrobatic Sex</title>

 <description>&lt;p&gt;Snap Crackle And Pop&lt;/p&gt;&lt;p&gt;&amp;nbsp;&lt;/p&gt;&lt;p&gt;&lt;br /&gt;There are some sounds you never want to hear while having sex. &lt;br /&gt;Snap, crackle and pop accompanied by pain in the penis is a sure indicator that you may have fractured the penis. Admittedly the penis is not a bone but with vigorous sex, especially accompanied by blunt trauma the connective tissue surrounding the penis can rupture.  The penis can accidentally hit the pelvic bone during vigorous sex tearing the connective tissue. As a result of sex in the shower the partner can slip resulting in a downward force on the erect penis fracturing the penis as well. A fractured penis requires immediate medical attention to preserve sexual function and avoid injury to the urethra (the tube through which urine flows).  The mechanism by which the penis ruptures can be so embarrassing that it delays medical treatment.  Unfortunately with alcohol or illicit drugs couples may not realize that acrobatic sex can be more risky than previously realized.  <title>Hardness Matters </title>

 <description>&lt;p&gt;How often are your erections hard enough to penetrate and were you able to maintain your hardness after you penetrated your partner, are critical questions necessary for most men ,to fully enjoy their sexual experience. Most men do not appreciate the subtle changes in the quality of their erections over time. Erectile dysfunction does not occur overnight, unless you have fractured your penis, which, I will discuss in a future blog. As a result of aging and lifestyle men have decreased blood flow and increased connective tissue in their penis resulting in decreased hardness.  In a worldwide study, men said that erection hardness is very important to having a good sexual experience. More than 50% of men are not fully satisfied with the hardness of their erections. In a survey of 600 men currently taking oral medication for ED identified &amp;quot;Enables me to have hard erections&amp;quot; as the most valuable attribute of ED therapy.&lt;br /&gt; The Erection HARDNESS Score was developed in 1998 as a tool to rate degree of erection hardness. &lt;br /&gt;Grade 1 penis is larger but not hard&lt;br /&gt;Grade 2 Penis is hard but not hard enough for penetration&lt;br /&gt;Grade 3 Penis is hard enough for penetration but not completely hard&lt;br /&gt;Grade 4 Penis is completely hard and fully rigid .&lt;/p&gt;&lt;p&gt;Women clearly can clearly sense the hardness of the penis and believe hard is good harder is better.&lt;br /&gt;Next week I will share with all of you what needs to be done to maintain a hard erection.  <title>Early Climax</title>

 <description>&lt;p&gt; In this follow/up blog on premature ejaculation a condition that affects 30% of men understanding the complex interaction of nerves in the penis spinal cord and complex areas in the brain it is remarkable  that any man achieves an erection or controls his ejaculation. We are learning of the special interaction between such neurotransmitters as serotonin dopamin and several others which seem to control the timing of ejaculation. There are clearly psychologic and biologic causes for premature ejaculation. Frequency of sex, sexual technique and newness of the partner may conrtribute as well as biologic issues such as erectile dysfunction, prostatitis and certain medications.&lt;/p&gt;&lt;p&gt;The best current approach is a combination of sexual techniques and serotonin drugs.  <title>Minute Men</title>

 <description>&lt;p&gt;Patriot Day was recently celebrated in Boston. However, minute men are never celebrating.&lt;/p&gt;&lt;p&gt;Timing is everything. Sex needs to be an orchestra when all the instruments come together. But premature ejaculation,  the most common sexual health complaint of men and the partners who love them, affects at least 30 percent of men, and it is a subject rarely discussed in the doctor&#039;s office by either men or women. &lt;/p&gt;&lt;p&gt;The strict definition you&#039;ll get from sexual medicine experts includes such scientific terms as &amp;quot;intravaginal latency period&amp;quot;—simply stated, the time it takes to ejaculate after penetration. For men with premature ejaculation, the time period can be as short as 30 seconds. For some, the ejaculation occurs before penetration. This condition has enormous consequences for the man and his partner, resulting in significant distress. &lt;/p&gt;&lt;p&gt;Just this week , two different patients, a young wife and and  single man, discussed the issue with me as part of their check up.  I am certain both will be helped. Doctors and patients need to be free to listen and appreciate that very effective treatments are available. The biology of premature ejaculation is starting  to be understood and it is important to appreciate it affects both young and older men. &lt;/p&gt;&lt;p&gt;In my upcoming posts, I will discuss new treatments, including a new medicine called Dapoxetene that affects the serotonin system. I will share techniques that have been helpful. But first, I invite all of you to share your experiences—success and failures. No shame, live stronger. &lt;/p&gt;</description>
